An English couple managed to win a lottery ticket in 1994. They won $5 million, but the couple lost the ticket.

The problem could have been avoided if the couple had reported the ticket missing in time. However, they were only able to do so after 30 days. Therefore, their win is considered to be the biggest win that was never picked up.
